Program Coordinator

Naugatuck Valley Health District is seeking a full-time, grant-funded COVID-19 Immunization Action Plan (CAP) Program Coordinator that will work primarily on the objectives of the COVID-19 Immunization Action Plan (CAP) contract.

The CAP Program Coordinator shall provide culturally competent COVID-19 immunization education and outreach services that targets individuals in underserved communities, communities in high social vulnerability index towns and other vulnerable populations in the Naugatuck Valley Immunization Action Plan jurisdiction, including the towns served by the Naugatuck Valley Health District (Ansonia, Beacon Falls, Derby, Naugatuck, Seymour, Shelton), as well as Oxford, Trumbull, Monroe and Stratford.

Employment is contingent on grant funding and will end on June 30, 2023. Continued employment beyond this date is contingent upon the availability of additional grant funding.

Compensation

This is a full-time position paid at $38/hour for 35-hours per week. Hours may include some required work performed on Saturday and Sundays. Primarily office work, although participation at community events and off-site work may be required. Remote work may be performed only with the permission of the Assistant Director of Community Health Services or Director of Health. This position is eligible for the benefits of a full-time employee consistent with the Personnel Policies of the Naugatuck Valley Health District.

Minimum Qualifications

The outreach worker must have a thorough understanding of the area’s population and the goals of the immunization program, parenting, or case management skills with new parents. Our ideal candidate will be communicate in a professional manner with state and local health departments, public officials, and the general public employing dynamic communication and interpersonal skills, cultural competency/sensitivity, tactful language, and empathetic interviewing skills to build rapport and maintain trust.

  • Must be able to handle a fast-paced, heavy workload environment and demonstrate excellent public service skills.
  • Ability to interact in a non-judgmental manner with culturally diverse populations and persons experiencing a wide range of social conditions.
  • Ability to use discretion, maintain confidentiality, and ethical conduct.
  • Ability to read and write English required. Bilingual (English/Spanish) desirable.
  • Knowledge of the approved COVID-19 vaccines.
  • Flexibility to work community events or NVHD events during some evenings or weekends.
  • Own transportation required for local and in-state travel.

Responsibilities/Job Description:

This position reports directly to the Assistant Director of Community Health Services. This position supervises one full-time and one part-time CAP Community Outreach Workers.

  • Conducts a COVID-19 Vaccine Confidence Rapid Community Assessment.
  • Increasing client compliance with established immunization schedule.
  • Providing parents and their children in with assistance in accessing the health care system.
  • Providing assistance in monitoring of the program outcomes.
  • Works with the Immunization Action Plan (IAP) in managing the CT Immunization Information System, CT WiZ.
  • Providing statistical reports to the Community Health Division’s Immunization Program.
  • Interactions with area vaccination providers to obtain required data and promote vaccine compliance.
  • Assists local health departments within NVHD’s IAP jurisdiction to identify gaps or challenges with vaccine compliance.
  • Develops immunization education materials, and promotes state, Federally, or otherwise recognized developed materials.
  • Contacting Primary Care Clinics, School Based Health Centers, Community-based and Faith-based organizations, and other similar organizations to increase referrals and compliance accordance with established plan of immunization.
  • Host community forums, focus groups, panel discussions, or other health education and promotion events related to immunizations.
  • Performing field outreach to maintain immunization plan.
  • Maintaining accurate documentation of all action on patient records and appropriate reports.
  • Participating in statewide COVID-19 Action Plan (CAP) and/or Immunization Action Plan (IAP) program meetings.
  • Monthly Reporting Metrics, Progress Reporting, and other deliverable record keeping.
  • Performs other duties as required.
