Community Outreach Worker

(Full-Time)

Naugatuck Valley Health District is seeking a full-time, grant-funded Community Outreach Worker that will work primarily on the objectives of the COVID-19 Immunization Action Plan (CAP) Program.

The Community Outreach Worker shall provide culturally competent COVID-19 immunization education and outreach services that targets individuals in underserved communities, communities in high social vulnerability index towns and other vulnerable populations in the Naugatuck Valley Immunization Action Plan jurisdiction, including the towns served by the Naugatuck Valley Health District (Ansonia, Beacon Falls, Derby, Naugatuck, Seymour, Shelton), as well as Oxford, Trumbull, Monroe and Stratford.

Employment is contingent on grant funding and will end on June 30, 2023.  Continued employment beyond this date is contingent upon the availability of additional grant funding.

Compensation

This is a full-time position paid at $30/hour for 35-hours per week. Hours may include required work performed on some Saturday and Sundays, or evenings. Primarily office work, although participation at community events and off-site work are sometimes required. Remote work may be performed only with the permission of the Assistant Director of Community Health Services or Director of Health.  This position is eligible for the benefits of a full-time employee consistent with the Personnel Policies of the Naugatuck Valley Health District.

Minimum Qualifications

The outreach worker must have a thorough understanding of the area’s population and the goals of the immunization program, parenting, or case management skills with new parents. Our ideal candidate will communicate in a professional manner with state and local health departments, public officials, and the public employing dynamic communication and interpersonal skills, cultural competency/sensitivity, tactful language, and empathetic interviewing skills to build rapport and maintain trust.

Responsibilities/Job Description:

This position reports directly to the COVID-19 Immunization Action Plan (CAP) Program Coordinator under the supervision of the Assistant Director of Community Health Services and the Director of Health.
