Food Service License Renewals due BEFORE 12/31

Food service renewal applications were mailed in November to individuals who have Food Service permits, which expire on December 31st. Additionally, a recorded phone broadcast message was also sent out to all license holders on November 24th with the same information.

Please return the completed application and the appropriate fee either by mail or in person BEFORE DECEMBER 31, 2021 to:

A late fee in the amount of 10% of the annual fee per day will be assessed for all applications not received by December 31, 2021.
